1. I have received a notification from CPSO that I am enrolled in the QI Partnership for Hospitals Program. Why am I receiving this notification?
Your name has been submitted as a confirmed participant by an approved organization in the QI Partnership for Hospitals Program. While your name has been submitted by your hospital organization to enroll you in the program, you are able to choose whether you would like to participate in this program option via Step 1 (Information Sharing Consent) within your QI Program homepage.
2. Do I need to provide proof of completion for each QI activity completed in Step 1?
No. You do not need to submit proof of completion for each of the QI activities you choose to complete in Step 1. Only the QI Activity Record and Reflection must be completed and submitted to CPSO. However, participants are encouraged to retain their certificates of completion in the event that CPSO performs a random audit, and for the purposes of CPD credits.
3. Can I pick any 3 QI activities I choose for the purposes of Step 1?
Participants must choose a minimum of 3 activities from the provided menu for the purposes of completing Step 1.
4. What can I do if I want to participate in QI Partnership Program?
Your hospital must be an approved organization for you to enroll in the QI Partnership program option. From there, a QI lead will need to submit your name to CPSO to acknowledge that you are an active participant within your hospital’s approved QI initiative. We encourage you to reach out to a CPSO Quality team member to discuss the options.
5. If I select “yes” on the Information Sharing Consent form to sharing information with my hospital, what information does CPSO share with my hospital?
CPSO may share the status of your completion of the required program activities and whether you requested to be withdrawn from the QI Partnership program option. Any reflections and information you submit as part of the required QI components of the program will not be shared with your hospital.
6. What do I do if my hospital’s QIP initiative hasn’t started yet?
The deadline for you to complete your program activities is unrelated to your hospital initiative and its timing. You can still complete the required QI activities and reflective activity questions. However, to do so, you do need to have knowledge and understanding of your hospital’s anticipated QI initiative, how you will be involved, and its potential impact on your practice.
7. I am a Quality Improvement (QI) leader at my hospital and would like my organization’s QI initiative to be approved for the QI Partnership for Hospitals Program and have physicians join the QI Partnership Program. What do I need to do?
If you and your organization would like to be part of the 2026-2030 QI Partnership Program, we encourage you to review the QI Partnership Information package and QI Partnership Proposal form to understand the associated criteria for participation and expectations. From there, please submit the QI proposal form for it to be reviewed by our QI Partnership leadership.
After your initiative is approved and your organization is confirmed as a QI Partnership participating organization, you can send physicians’ names for enrollment in your hospital’s QI Partnership program.
8. How can I change my QI deadline in the Portal?
Please reach out to a Quality team member if you have questions about your deadline or if you feel you need more time to complete the QI program requirements.
9. What if I’ve previously completed one of the listed QI activities? Can this count as one of my QI program activities?
If you completed one of the modules listed on our QI Activity Options within the last 6 months, you may count it towards your QI Program participation. Remember to retain a record of your certificate of completion in the event that CPSO requests it.
10. Where can I provide feedback about my QI program experience?
Your perspectives on the QI Program components and format are exceptionally important to us. Following the completion of Steps 1 through 3, we ask that participants take a moment to provide feedback using the link available within their QI Program page in the CPSO Portal.
