Membership Diploma

This diploma certifies membership with the College and is designed for framing and displaying in your office and may be ordered by any member holding an independent practice certificate of registration.

Cost: $75

To purchase your membership diploma, complete this interactive Order Form, print out, and mail with your payment to the College.

Payment can be made by Visa, MasterCard or cheque, and must be submitted with the order form. If paying by credit card, please include your card number and expiry date on the order form. Please allow up to 12 weeks for processing, and delivery by courier.
